Support Time and Recovery Worker - SECONDMENT
12-month fixed term post to cover maternity leave within the Mental Health Intensive Support Team (MhIST).
OverviewThis role provides intensive rehabilitation support to people with complex mental health needs to help regain skills and confidence for independent living, incorporating dialogical/Open Dialogue approaches.
Responsibilities- Support the MDT in delivering intensive rehabilitation to a small cohort of patients and ensure high-quality care to promote independence.
- Collaborate with Primary Workers to deliver needs-led care with a focus on dialogical practice, recovery, identifying relapse indicators, skill development and promoting community participation.
- Support engagement in the agreed care plan and encourage inclusion in local community services and daily living skills.
- Demonstrate willingness to develop skills in dialogical practice/Open Dialogue.
- Participate in training and support for staff and external organisations (e.g., Supported Living or Care Agency staff) in patient care.
- Proactively contribute to risk management and care planning to prevent crises and reduce hospital admissions, with effective communication across scenarios.
- Work with a range of health and social care backgrounds; experience with complex patients is encouraged.
- Recent relevant experience supporting and integrating people with mental health issues toward recovery.
- Experience working in a Mental Health community setting, rehab setting, or equivalent.
- NVQ Level 3 in Health & Social Care or equivalent.
- Understanding of Community Rehabilitation and Recovery Models and their application.
- Experience in producing and leading teaching and learning in group settings.
- Experience of working with clients in recovery from serious mental health issues.
