Job Purpose:This role plays a vital part within Brio’s Wellbeing Team, supporting residents across Cheshire West and Chester to improve their health, reduce health inequalities and access services that help them make positive lifestyle changes.
As a Health Referral Advisor, you will provide a professional, compassionate and person-centred triage service for residents referred into Brio’s wellbeing programmes, including smoking cessation, weight management and falls prevention.
As the first point of contact for many residents, you will support people from a wide range of backgrounds, including those who may be vulnerable, residents requiring language interpretation or translation support, residents living with long-term health conditions or those who face barriers to accessing services. Through effective listening, motivational conversations and a non-judgemental approach, you will help people understand their options and take the first steps towards improving their health and wellbeing.
Health Referral Advisors provide the vital link between residents seeking support and the teams delivering Brio’s programmes. By managing referrals, assessing eligibility, coordinating appointments and supporting residents throughout their journey, they help ensure people can access the right support at the right time.
You will work closely with colleagues across the Wellbeing Team, wider Brio Leisure services, primary care, secondary care, social care and community organisations to provide a seamless and positive experience for residents accessing wellbeing support.

Main ResponsibilitesProvide a professional, welcoming and compassionate first point of contact for residents accessing Brio's wellbeing services via telephone, email, digital platforms and face-to-face interactions.
Assess client needs through effective conversations, ensuring a thorough understanding of individual circumstances, support requirements and any barriers to engaging with services.
Coordinate referrals into the appropriate wellbeing programme, ensuring residents are supported throughout the process and receive a positive customer experience.
Identify and implement reasonable adjustments where appropriate to support equitable access to services and improve engagement for individuals with differing needs.
Demonstrate a culturally aware, inclusive and non-judgemental approach when supporting residents from diverse communities and backgrounds.
Support people who may be vulnerable, socially isolated or experiencing multiple challenges by providing guidance, reassurance and appropriate signposting.
Ensure the effective operation of booking procedures, including enrolment, assessments, inductions and one-to-one appointments.
Maintain accurate and confidential records, ensuring all information is recorded in accordance with data protection requirements and service procedures.
Update referral databases and associated systems with required information to support service monitoring, performance reporting and evaluation.
Liaise with programme providers and delivery staff to ensure they have the information required to provide safe, effective and person-centred support.
Conduct programme induction requirements by telephone, online or face-to-face in accordance with referral pathways and service procedures.
Proactively maintain high standards of customer care and client engagement to support attendance, retention and successful outcomes.
