Cheshire & Wirral Partnership NHS Foundation Trust seeks a full-time Mental Health Practitioner to join our Crisis Resolution Home Treatment Team in Chester.
You will work within a multidisciplinary team to assess people in crisis, deliver interventions, and coordinate care across Urgent Care First Response services including liaison services, street triage, liaison and diversion services, and the crisis line.
