The Countess of Chester Hospital NHS Foundation Trust is seeking a Community Support Worker to join the Chester Community Response Hub, delivering an early supported discharge service for patients from the Countess of Chester Hospital and Ellesmere Port Hospital.
You will work shifts across 7 days between 8am and 9pm, travel to patients at home, assist with activities of daily living and support therapists and nurses in caseload management. A full driving licence and car are required.
