Job overview

A fantastic opportunity has arisen for a Band 6 Mental Health Lead Nurse, or an experienced Band 5 looking to take the next step into a Band 6 role, to join the Older Adult In-Reach and Home Treatment Team in Chesterfield. The primary aim of the service is to support older adults experiencing a deterioration in their mental health within their own homes and, where appropriate , avoid hospital admissions where appropriate.

The team also works closely with mental health inpatient wards to facilitate timely discharge and support patients in returning home as soon as it is safe to do so. The service operates 7 days a week, from 9:00am to 5:00pm, including weekends and bank holidays. Due to the large geographical area covered by the service and the requirement to respond within 2–4 hours, the successful candidate must be able to travel independently, with access to their own vehicle being essential.
