Derbyshire Healthcare NHS invites applications for a Band 6 Lead Nurse in the In-Reach and Home Treatment Team for Older Adult Crisis in Chesterfield. The post is full-time (37.5 hours) and permanent, based at Scarsdale Hospital, with a patient-focused remit to support older adults in their homes and prevent unnecessary hospital admissions.
You will work within an MDT, coordinating with GPs, social care and mental health teams.
