Job summary

The Brain Injury Clinical Specialist (OT) will work in partnership with team members, service users, DCHS staff, D&D ICB, GP and other agencies to provide effective evidenced based rehabilitation and care to people with Acquired Brain Injury (ABI) and Traumatic Brain Injury (TBI). Achieved through effective clinical management of own caseload, provision of specialist therapeutic advice to colleagues outside the specialist team, effective delegation, case load management.

Community Caseload:
To identify the advanced therapy needs and provide treatment to individuals within the North Derbyshire area. To work flexibly across the area in different care settings providing a patient led service. Co-ordinate referrals to other agencies and to support other therapy staff in neighbouring localities with complex cases as and when required.

Complex Caseload:
To provide expert case management on behalf of D&D ICB - assessing, planning, supporting treatment and evaluating clinical reasoning - across Derbyshire and Derby City including acute hospitals, Regional Dedicated Brain Injury Specialist Units, Community Units, Service User's home, local community and Clinic settings. Provide high level reports and feedback as required or requested.

Main duties of the job Community Caseload

To undertake the comprehensive assessment of patients who have highly complex presentations, using advanced clinical reasoning skills and assessment techniques to provide an accurate formulation of their condition.

To formulate and deliver personal care plans and individual treatment programmes based on sound knowledge of evidence-based practice and treatment options, using clinical assessment, reasoning skills and treatment skills e.g exercise therapy, patient education, cognition assessment, identification and prescribing of specialist equipment, and other techniques as appropriate.

Complex Case Management

To provide a case management service to ensure that the clinical needs and outcomes of patients with an acquired Brain injury who have an individual specialist rehabilitation placement approved by D&D ICB decision making panel, are met in a safe and effective way.

To be able to travel widely across the country to provide this case management service.

Provide advice and support for funding panel decision making.

Provide oversight of D&D ICB funded placement ensuring safe and effective rehabilitation is being provided, supporting decision making and monitoring patient progress.

Support and oversee effective discharge from placement facility including funding streams, care and equipment needs.
