Polaris Rehabilitation & Care Center is seeking a dependable, compassionate PRN CNA Transportation Aide to help residents travel safely to medical appointments. Enjoy an opportunity that combines resident care, meaningful connections, and as-needed scheduling.

• Safely drive residents to appointments and other approved destinations.
• Assist with transfers, wheelchair securement, mobility, and personal care within your CNA scope.
• Coordinate appointment schedules and paperwork with Nursing.
• Monitor resident comfort and promptly report changes or concerns.
• Complete transportation logs, vehicle safety checks, and routine vehicle cleaning.
• Provide additional CNA support when transportation assignments permit.

• Active Wyoming CNA certification in good standing
• Valid driver's license
  , acceptable driving record, and ability to meet facility vehicle-insurance requirements
• Safe transfer and mobility-assistance skills
• Dependability, patience, and strong communication
• Ability to complete required background checks and training
• Transportation or wheelchair-accessible vehicle experience preferred

PRN / As Needed. Hours depend on resident appointments and facility needs, with weekday availability preferred. Hours are not guaranteed.
