Registered Nurse – Home Health (Full-Time)
What You’ll Do
- Conduct patient assessments and ongoing re-evaluations
- Develop and manage individualized plans of care
- Provide skilled in-home nursing services
- Complete required documentation, including OASIS and care summaries
- Coordinate care with physicians and interdisciplinary teams
- Educate patients and families on health management
- Participate in on-call coverage as scheduled
Qualifications
- Graduate of an accredited nursing program
- Active RN license in the state(s) of practice
- 2+ years of nursing experience preferred
Schedule
- Full-time (40 hours/week)
