Frontier Hospice is seeking a Per Diem Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end‑of‑life care throughout the community.

- Respond to clinical referral information with care and expertise.
- Partner with the Clinical Director to assess eligibility and suitability of clients.
- Serve as a source of comfort and support for patients, ensuring their comfort and quality of life.
- Deliver top‑notch nursing care to ease pain and bring solace.
- Educate and advocate for patients and families, encouraging participation in personalized care plans.
- Participate in call duties as needed.
- Current, unencumbered Registered Nurse license.
- Minimum one (1) year of RN experience in a medical‑surgical/acute care setting (hospice experience a plus).
- Computer proficiency, including EMR familiarity.
- Current CPR certification if required by state.
- Valid driver’s licence, vehicle insurance, and reliable transportation.
$36.00 – $40.00 per hour (average). Final compensation will be based on experience, qualifications, and responsibilities and will be confirmed at the end of the interview process.
