Health Continuity Specialist - Chico Concord
Position Summary:
Under general supervision from the Community Health and Outreach Director and providers, the responsibilities of a Health Continuity Specialist are to support patients with referrals and coordinate care to improve health outcomes for NVIH patients as measured through Quality Programs such as HEDIS and GPRA.
- Utilize reporting platforms to review patient records for accuracy across reporting agencies/providers and identify gaps in care.
- Engage in outreach to patients to improve health outcomes by identifying gaps in care, providing education about preventive health screenings, sharing resources to remove barriers, and following up on appointments and referrals.
- Coordinate with medical staff to inform them of missing screenings and gaps in care.
- Ensure GPRA and HEDIS are updated with accurate data for services and screenings completed, discovered through researching and auditing of reports and records.
- Participate in CQI meetings and provide support for process improvement initiatives.
- Other duties as assigned.
- High school diploma or GED equivalent; degree in healthcare administration or clinical medical profession preferred.
- Previous experience in healthcare administration, patient navigation, care coordination, or case management.
- Knowledge of healthcare systems, medical terminology, and various healthcare settings to navigate the complexities of patient care and coordination.
- Works independently and takes initiative.
- Teamwork and collaboration.
- Communication skills at various levels; bilingual preferred.
- Detail-oriented and problem solving.
- Proficient with EHR and computer systems.
- Cultural sensitivity in navigating challenges in vulnerable groups.
- Reliable transportation and ability to adhere to regular attendance.

Salary$13,224.00-$15,211.00 annually.
