We’re looking for a Licensed Practical Nurse (LPN) to join our team in the MHA GRIT Program, part of the Division of Recovery & Housing. This is a unique role where nursing care meets recovery-focused support—designed for someone who wants to make a difference while working in a team that truly collaborates across disciplines.

In this role, you won’t be working in a hospital or traditional clinic—you’ll be embedded in a program that provides housing and structured support for individuals working to rebuild their lives after serious mental health challenges and/or substance use. You’ll have the opportunity to develop meaningful relationships with participants, offer hands-on clinical guidance, and be a consistent presence in their care and recovery journey.

Whether you're supporting medication management, helping coordinate medical care, or simply being a steady, trusted voice in someone’s day—your role will matter. And you'll be working alongside a committed, values-driven team that brings empathy, skill, and accountability to everything we do.

Shift:

-Tuesday through Saturday 9am-5pm (40h)What You’ll Be Doing
  • Assess the physical health needs of program participants and help shape individualized care plans
  • Monitor medications, observe side effects, and provide clear education to participants about their treatment
  • Communicate regularly with outside providers, physicians, and VNAs to coordinate care
  • Support a culture of wellness through participant engagement and informal health coaching
  • Partner with staff on MAP (Medication Administration Program) training and compliance
  • Be part of the team that ensures participants get timely, quality healthcare—including advocating for follow-up care and second opinions when needed
  • Participate in treatment planning, discharge planning, and program wellness initiatives
  • Keep clear, compliant, and organized documentation on all participant health services

  • Comfortable educating both participants and staff on medical and medication-related topics
  • Organized, good under pressure, and skilled at keeping multiple tasks moving

  • LPN license in Massachusetts is required
  • At least 2 years of experience working with individuals with psychiatric or behavioral health needs
  • Knowledge of psychotropic medications, common medical conditions, and wellness principles
  • MAP instructor certification or willingness to become certified
  • Strong communication skills, both verbal and written
  • Ability to coordinate care and advocate effectively within the healthcare system
