• Guaranteed salary of $88,000- $97,000, based on experience, plus a productivity bonus

• You’ll guide patients through their home health care, from assessment to discharge.
• That includes providing skilled nursing visits, developing and updating care plans, managing an assigned caseload, and coordinating with physicians and other clinicians.
• You’ll also teach patients and caregivers how to manage medications, treatments, and health conditions at home.

• Current, unrestricted Massachusetts RN license
• At least one year of clinical nursing experience
• Strong assessment, communication, and time management skills
• Ability to work independently and travel throughout Springfield
• Valid driver’s license, current auto insurance, and reliable transportation
• Ability to sit, stand, bend, lift, and move intermittently and lift 50-100 pounds
