• Monday, Thursday, and Friday 8 a.m. to 5 p.m. On-call 1-2 times weekend and every 4th-5th weekend.
• Coverage area will include the counties of Harrison, Grundy, Livingston, Davies, Caldwell, Mercer, and parts of Sullivan, Linn, Chariton, and Carroll.
• Work one on one with patients.
• Work autonomously as a RN.
• Perform nursing assessments in the patients’ home.
• Utilize full scope of nursing skills, including wound and ostomy care, PICC line care, chronic care education, and medication reviews.
• Coordinate patients’ care – consulting other disciplines of the health care team.
• Individualize patient care plans and adjust or modify the nursing plan of care as appropriate.
• Collaborate with the medical staff and other disciplines to provide and enhance patient care.

• Missouri and Kansas RN License required.
• Experience:
  
  Less than 1 year.
• Education:
  
  Associate Degree.
• Registered Nurse (MO) – National Council of State Boards of Nursing (NURSYS).
