Home Health Physical Therapy

Location: Chillicothe, OH

Shift Details: Monday-Friday, 8hr shifts, 40 hrs a week, no weekends/holidays

• Must have State Physical Therapist License
• Must meet productivity expectation of 30 points per week
• 1 year of home health experience preferred, in the last 3 years
• HCHB experience preferred

• Patient Assessment:
  Conducts thorough evaluations to determine the patient's needs and goals.
• Treatment Plan Development:
  Creates individualized therapy plans, often in consultation with physicians.
• Treatment Implementation:
  Provides hands‑on therapy, exercises, and education to patients.
• Documentation:
  Maintains accurate records of patient progress and interventions.
• Communication:
  Communicates with physicians and other healthcare team members to ensure coordinated care.
• Patient and Caregiver
  
  Education:
  
  Provides education and counseling to patients and their families about their condition and treatment.
• Supervision:
  May supervise physical therapist assistants, in accordance with applicable regulations, according to Indeed.
• Progress Monitoring:
  Regularly evaluates patient progress and adjusts treatment plans as needed

• 13 week contract with possible extensions
