Overview
We are looking for a Family physician with advanced training in Care of the Elderly to join our team in Fraser East, affiliated with Fraser Health Authority.
Responsibilities- Provide care to home health and homebound seniors in the Chilliwack community, including home visits and liaison with the home and community care team and the client’s family physician.
- Collaborate in an interdisciplinary setting; may contribute to transitional care (hospital to home) to improve outcomes and reduce readmissions.
- Participate in academic and teaching components of the Service and pursue a clinical faculty appointment with the University of British Columbia.
- Advanced training in Care of the Elderly (certification preferred)
- Strong collaborative interpersonal and inter-disciplinary skills
- Experience in Community Geriatric consultations
- Eligibility to apply for a UBC Clinical Faculty Appointment
- Part-time position (0.2 - 0.3 FTE)
