Your role within the Intensive Case Management Team will allow you to apply a trauma-informed approach to care. You will provide comprehensive assessments, clinical interventions, and client advocacy while maintaining cultural humility. This position emphasizes a community-centered model of care, connecting clients to essential resources and promoting their well-being.

Key Responsibilities:

• Conduct screenings and assessments for Indigenous clients

• Provide initial interventions and ongoing support

• Collaborate with community agencies for resource access

• Educate clients and families using a culturally relevant lens

• Ensure accurate documentation in clinical systems

Requirements:

• Master’s Degree in a related social or health program

• Two years of experience in mental health services

• Knowledge of Indigenous health protocols

• Valid driver's license for work-related travel

• Strong assessment and organizational skills
