Fraser Health Indigenous Health Social Worker

This position plays a pivotal role in assessing and addressing the psychosocial health needs of Indigenous clients. Key Responsibilities:

• Serve as a liaison for clients, families, and healthcare providers

• Administer assessments covering diverse health aspects

• Develop and deliver health education programs

• Facilitate connections with community health services

• Maintain accurate client documentation and reporting

Requirements:

• Bachelor's degree in Social Work from an accredited institution

• Two years' related experience, including work with Indigenous communities

• Registration with the BC College of Social Workers

• Valid BC Driver's Licence and personal vehicle for travel

• Comprehensive knowledge of Indigenous cultural practices
