Detailed Overview

The Indigenous Care-Giver Support Model supports clients in their homes, through receiving care from trusted family and community members (known as care-givers) with a focus on culturally safe and appropriate care. The model helps care-givers build their skills to support their loved ones with care tasks such as toileting, bathing, turning/positioning, etc. Care-givers work directly with the Indigenous Care-Giver Support team, including the Licensed Practical Nurse who provides education, and ensures connection to the Home Support Nursing team should further or additional clinical needs arise.

In addition to supporting care-givers, the Licensed Practical Nurse performs nursing procedures within the competency guidelines and standards of practice as outlined by the British Columbia College of Nursing Professionals (BCCNP) and the current legislative scope of practice in a community health setting; coordinates the provision of support services and liaises with Home Health and Indigenous Health staff, as required.

Responsibilities
  • Visit assigned care-givers in the client’s home to assess the skills and abilities of the care-givers to meet the client’s needs; maintain records of such visits; complete and maintain related records and documentation such as progress reports, observations and client charting; report and respond to problems such as care issues, complaints, safety concerns by investigating and taking required action.
  • Support designated care-givers by determining training and orientation requirements and orienting new care-givers.
  • Assess and evaluate care needs of the client in accordance with competency guidelines, established standards of practice and current legislative scope of practice; share information and knowledge, provide direction, assignment and support to care-givers, as appropriate.
  • Contribute to the development of the client’s care plan in collaboration with the Indigenous Health Clinical Resource Nurse. Provide feedback to the established clinical support care team including Home Health, First Nation Health providers, and other allied health services supporting the client and family; meet with the care-givers regularly to provide emotional support and feedback.
  • Contribute to the development and revision of standards of care, policies and procedures and advocate improvements in clinical nursing practice, health care and health care services; maintain department standards and procedures.
  • Perform nursing care and procedures and evaluate client outcomes; record observations and report problems and/or changes to designated care-givers for follow up; administer medications at assigned clients’ homes according to established policies and procedures, including medication reminders.
  • Respond to general inquiries by telephone and in person; provide direction and information about available programs and policies.
  • Assist in the selection and identification of appropriate care-givers in collaboration with families, community supports, and the clinical care team.
  • Perform other related duties as assigned.
Qualifications Education, Training and Experience

Graduation from an approved school in Practical Nursing. One year recent, related clinical and/or mentoring experience in a community health setting, or an equivalent combination of education, training and experience. Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nursing Professionals (BCCNP). Valid BC Driver’s License and access to a personal vehicle for business-related purposes.
