The salary range for this position is CAD $41.42 - $59.52 / hour
Job Summary
Fraser Health is responsible for the delivery of hospital and community-based health services in British Columbia. The Registered Nurse provides direct patient care by assessing, planning, providing and evaluating nursing care to patients in acute inpatient areas, in collaboration with the interprofessional care team.
Responsibilities
- Assesses patients and collects information from a variety of sources using observation, communication, data review and physical assessment. Makes a nursing diagnosis and plans, performs and evaluates nursing interventions and care.
- Collaborates with patients and families in developing the care plan and encourages participation in its development, implementation and ongoing modification. Maintains therapeutic relationships and advocates for patient rights to privacy, dignity and information access.
- Participates in care and discharge planning to ensure effective implementation and coordination with the interprofessional care team.
- Coordinates nursing care for the patient; sets priorities, assigns and delegates tasks to support staff, and makes referrals per established guidelines.
- Collaborates with the interprofessional care team to identify and resolve issues and adjust care plans as patient conditions change.
- Educates patients and families about illness prevention, health restoration and maintenance; assesses learning needs and provides information to support self-care and discharge planning.
- Plans referrals to appropriate health care/community agencies in collaboration with the care team.
- Documents assessments, diagnoses, interventions, patient responses and outcomes according to practice standards and Fraser Health procedures.
- Participates in committees, care conferences and quality improvement activities as assigned; maintains clinical competence and pursues ongoing learning.
- Acts as a preceptor or mentor to new nursing staff and students; contributes to orientation and sharing of knowledge.
- Performs other related duties as required.
Qualifications
Education and Experience
Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
Skills And Abilities
- Ability to complete initial and ongoing patient assessment and provide nursing care with appropriate technical, therapeutic and safety interventions.
- Excellent communication with patients, families, the public, medical staff and the interprofessional care team using verbal, written and electronic means.
- Commitment to person-centered care, empathy, cultural safety and patient advocacy.
- Ability to organize, prioritize and collaborate with the care team; use evidence-based practice to inform nursing decisions.
- Ability to teach patients and families about essential health topics; proficiency with relevant computer applications and equipment.
- Physical ability to perform the duties of the position.
