Job Description
The Éyameth’ Health Centre will offer safe, accessible and culturally appropriate primary care services to all residents North of the Fraser River between Agassiz and Mission.

The vision for Éyameth’ includes a medical clinic housed with a center of excellence, weaving together innovative, transformative health and wellness services that are driven by the community to address health care gaps in the region.

The Éyameth’ Health Center is a society-run, Nations-led Primary Care Center under the provincial First Nations Primary Care Initiative. We are a groundbreaking, standalone primary care facility partnered with the Ministry of Health, Fraser Health, FNHA and the Chilliwack Division of Family Practice.

Éyameth’ Health Centre services are designed to provide wraparound, team-based primary care, mental health and traditional wellness services for all residents of the North Fraser region. There will also be a home hemodialysis unit, pharmacy, level 1 lab and rehabilitation room. It is expected that the Centre will initially operate Monday to Friday (excluding Statutory Holidays) during regular business hours (this may change over time due to operational needs).

The Nurse Practitioner is expected to collaborate in team-based care for patients and support the practice of Western and Traditional Wellness approaches to patient health. A minimum panel size is expected to reach 600 patients by Year
3. Estimated Yearly Remuneration for the current service contract compensation is $171,533 - $182,968 per 1.0 FTE over 3 years based on panel attachment (Note- position is 0.5 FTE). Applicants may be eligible for additional incentives.

Responsibilities
The Nurse Practitioner (NP) will be responsible for practicing with cultural humility, providing primary care in a culturally safe, team-based environment that recognizes the physical, mental, emotional, and spiritual aspects of the client (and family) to provide whole person care, in compliance with policies and procedures of acceptable BC health and wellness regulations and standards.

Core Services in compliance with policies and procedures of acceptable BC health and wellness regulations and standards:

Current practicing registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM).

Recent relevant clinical nursing experience is considered an asset.

Culturally safe care integrating cultural humility, traditional values and ways of being for patients.

Serves as a team member with others such as General Practitioners and allied health staff as well as traditional wellness advisors who collectively provide primary care to the patient.

Ability to communicate and collaborate with patients/clients/residents and families about health findings, diagnosis, treatment, self-care and prognosis.

Diagnoses and treats previously undiagnosed patients/clients/residents for undifferentiated diseases, disorders, and conditions within the Nurse Practitioner's scope of practice; writes orders for treatment and medications; provides…
