Position: Social Worker for Long Term Care

Key Responsibilities:

• Conduct admission interviews and handle necessary documentation

• Liaise with agencies for residents' financial needs

• Provide ongoing counseling and education to residents and families

• Organize and lead care plan conferences with the team

• Document assessments and interventions accurately

Requirements:

• Bachelor’s degree in social work

• Minimum one year of experience in a hospital setting

• Registered with BC College of Social Workers

• Strong planning and prioritization abilities

• Physical capability to fulfill job duties
