About the Program

The Traverse program in Chilliwack provides bed-based treatment for youth (ages 13-18) whose lives have been impacted by substance use. Traverse is a recreation-based program using a therapeutic approach focusing on trauma-informed practice to provide treatment and basic care, service planning and therapeutic recreation to respond to the emotional and practical needs of youth.

Position Summary

Wage Grid 13

The Case Management Worker works as part of a large multi-disciplinary team that provides substance use treatment for youth (13-18) in a bed based, live in setting. As a member of the treatment team, the case manager understands the impact of substance use and mental health on youth and their families. This position will be the main point of contact for participants, families and community supports before, during and after treatment to ensure consistent service planning and continuum of care.

Responsibilities
  • Provides consultation for referral screening, assessment for program fit by using psychosocial assessment, initial counselling and basic interviewing skills
  • On participant admission, provides the following: aids in the completion on-site intake, updates electronic clinical record as required; provides facility orientation, assigns treatment teams, schedules care team meetings and visits
  • Participates in case conferences/meetings with treatment team to identify and support participant individualized service plans
  • Collaborates with the team to facilitate management and participants progression through the program; coordinates care using a supportive case management approach within the principles of harm reduction
  • Supports participants to manage self-care, develop social and emotional skills, and prevent relapses through supportive coaching, demonstrations, modeling and behavior management
  • Supports participants families and community support to access community services appropriate to the treatment planning of other participants
  • Communicates frequently and engage in team meetings, program planning and evaluation
  • Supports planned, early and administrative discharges
  • Encourages socialization and support among program participants; works with participants to promote informed decision making and to improve their quality of life
  • Researches and liaises with community services, consults with participants with regards to treatment planning and provide referrals to appropriate community services
  • Utilizes available community resources such as volunteers, community groups, alternate care services and other service agencies to develop supportive aftercare programs youth and their families
  • Contributes to team building and program and policy development by participating in regular team meetings, peer review processes and clinical rounds
  • Initiates and participates in individual professional development; maintains current knowledge and skills in relation to substance use, dual diagnosis, and youth and family issues; keeps informed of community resources
  • Performs other related duties as assigned
  • Demonstrated case management, assessment and counselling skills
  • Ability to communicate effectively, empathically and respectfully both verbally and in writing
  • Ability to establish rapport with participants, families, and members of the interdisciplinary team, and to effectively network with other services and agencies
  • Ability to prioritize and organize changing caseload needs and work effectively under pressure
  • Ability to work independently with minimal supervision
  • Ability to use computers and related software applications
  • Knowledge of addictions and substance use issues
  • Knowledge of community resources and how to access them
