Case Manager for Youth Programs

Empower youth at the Traverse Treatment Centre in Chilliwack as a Case Manager. This role focuses on providing essential support for individuals facing substance use challenges in a therapeutic environment.

As a Case Management Worker, you will work full-time alongside a diverse team dedicated to enhancing the lives of youth affected by substance use.

Your role includes conducting assessments, coordinating care, and fostering connections with community resources. Key skills include a thorough understanding of substance issues and effective communication with participants and their families.

Key Responsibilities:

• Conduct psychosocial assessments for program fit

• Manage and update participant care plans

• Assist in team planning and participant progression

• Encourage social interactions and informed decision-making

• Facilitate community service access for families

Requirements:

• 2-year diploma in a related field (required)

• Minimum 3 years experience in case management

• In-depth knowledge of harm reduction practices

• Strong interpersonal and communication skills

• Ability to manage caseload independently
