Position: Social Worker for Elderly Care

With a Bachelor's degree in social work and one year of recent experience in a hospital, you will conduct psycho-social assessments and crisis intervention, ensuring that residents receive holistic care. Collaborate within an interdisciplinary team to uphold high standards of service.

Key Responsibilities:

• Facilitate admission processes and manage documentation

• Assist families with financial and resource navigation

• Provide support and counseling services pre-admission to discharge

• Lead and chair care-plan conferences for coordinated care

• Maintain detailed records of services and advocacy efforts
Requirements:

• Bachelor’s degree in social work

• One year of related hospital experience

• Registered with BC College of Social Workers

• Excellent communication and interpersonal skills

• Capacity to work both independently and within a team
