Chipping Norton Health Centre has a long and proud history of providing high-quality,patient-centred care to our community. We are based in a modern, purpose-built building, which we moved into in 2015, complete with an on-site dispensary and excellent facilities.
The role advertised is for a Care Navigator to join our existing team of 12. The role is around 30 hours per week and involves shift patterns between the hours of 07:45 and 18:30 Monday - Friday.
Main duties of the job- Handle inbound and outbound calls courteously, efficiently and effectively.
- Handle inbound Triage requests inline with protocol.
- SUPPORT the Total Triage clinical Team.
- WELCOME patients and visitors to the Health Centre.
- SUPPORT the wider health centre team, including clinicians and admin staff to ensure first class service.
- COMMUNICATE with health care professionals, patients and non-clinical colleagues to ensure the smooth running of the health centre.
Chipping Norton Health Centre isa modern, purpose-built GP practice in the Cotswolds serving approximately 16,000 patients, formed from a 2015 merger of local surgeries. It offers comprehensive primary care, including a dispensary for rural patients, with a large, multidisciplinary team comprising GPs, nurses, and allied health professionals focused on patient-centered care.
Job responsibilitiesKey Responsibilities
- Patient Navigation:
Greeting patients and managing reception. - Administrative Support:
Processing online requests (Total Triage), managing appointment systems, inputting data into patient records and handling patient queries. - Communication:
Acting as the first point of contact for patients, dealing with queries, and facilitating communication between patients and clinical staff. - Support Services:
Assisting with medical administration - Clinical Support:
Working closely with and supporting the clinical team.
- Excellent verbal and written skills for navigating patients to appropriate services.
- Interpersonal
Skills:
Empathetic, calm, and able to handle stressful or distressing situations. - Administrative Ability:
Accurate data entry and computer skills (MS Office/systems). - Problem-Solving:
Ability to interpret patient needs and find solutions. - Flexibility:
Adaptable to changing needs and working as part of a multi-disciplinary team. - Previous experience within a health care based service is desirable.
- Experience of working within a fast paced, changeable environment would be beneficial.
- Previous experience working with EMIS is also desirable.
- Experience in a customer-facing role or administrative environment.
- Desirable:
Prior experience in a GP practice, NHS, or health/social care setting. - Qualifications:
Good general education (GCSEs, NVQ Level 2 or 3 in Customer Service or Health and Social Care). - Experience of working within a fast paced, changeable environment
