Physical Therapist

Full Time, Part Time, or PRN positions available

Assist in planning, organizing, developing, and directing Physical Therapy services in accordance with current federal, state, and local standards, guidelines, and regulations, and as directed by the Administrator, to assure the highest quality resident care.

• Provide physical therapy services to residents per established standards and company procedures.
• Document and bill services per company procedures.
• Evaluate, plan, implement, and follow up on resident therapy programs with sound judgment.
• Conduct regular screenings to determine need for intervention.
• Evaluate residents to obtain data necessary for treatment planning.
• Conduct specialized evaluations as indicated.
• Develop treatment plans with short‑ and long‑term goals.
• Communicate evaluation findings, treatment plans, and recommendations to residents, families, and interdisciplinary team.
• Perform other duties as assigned to discharge the responsibilities of the position.
• Complete compliant and medically necessary documentation in Net Health EMR.

• Minimum Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
