Out-of- Community Nurse

Lancashire and South Cumbria NHS Foundation Trust is seeking a Community Staff Nurse to join the Out of Hours Team. The role involves delivering high-quality care to patients in their own homes, carrying out holistic assessments, wound care, and palliative/end-of-life care in a seven-day rota, with support from the District Nursing Team.

You'll work with in a multidisciplinary environment, leading care planning, implementing programmes, and collaborating with other health and social care partners
