Hospital at Home are looking for a staff nurse who is motivated and enthusiastic to join our developing service and multidisciplinary team, with a passion for acute community care.

The role will include assessment and implementation of plans of care to patients whose conditions and treatment can be met within their own home or within community settings, to prevent them from remaining in acute hospital settings when appropriate.

A valid UK driving licence is essential for this role

This role requires a current NMC registration

Based at:
Christchurch, although travelling is required on a daily basis throughout the BCP locality. Previous NHS experience is a requirement due to the autonomy of the role.

Interview date: week commencing 13th July 2026

Responsibilities

Provide comprehensive, person‑centred holistic care to older adults in community and home settings, including assessment, care planning, medication administration, and escalation of deteriorating patients.

Deliver end‑of‑life care and support for patients and families, maintaining high standards of quality and safety in accordance with the NMC Code.

Experience
  • Demonstrable clinical experience
  • Experience caring for patients with acute and long‑term conditions for older people
  • Experience in providing end‑of‑life care
Knowledge
  • Fundamental nursing knowledge and practice in caring for the older person
  • Understanding of professional expectations and accountability in practice in line with the NMC Code
  • Awareness of role in relation to patient experience, quality and safety agendas that support governance frameworks
Qualifications
  • NMC registered Nurse
  • Advanced Diploma / Degree in adult health nursing studies
  • Teaching qualification
  • Post‑registration course related to care of the elderly
  • Literacy and numeracy competence
  • Demonstrable ability in management of vital signs monitoring and escalation of the deteriorating patient
  • Non‑IV medication administration
  • Venepuncture and cannulation
  • ECG recording
  • Awareness of audit in practice
  • IV medication administration including blood products
Other requirements
  • Use of own car for transportation, with a valid UK driving licence

Staff recruited are expected to be available to work flexibly across all our Trust locations to meet service demands.
