We are currently seeking a FT RN Case Manager for our New River Valley based service area.

As a RN Case Manager, you will coordinate, guide and direct the care of patients facing life‑limiting illness. You will work with the interdisciplinary Group to develop a plan of care, educate the patient and patient’s family in the care of the patient, and make visits to the patient’s home as frequently as necessary. We are seeking individuals who are passionate about clinical excellence and who feel a professional calling to caring for patients with serious and complex illness at end of life.

Average case load: 12‑14 patients.

The schedule will be Monday‑Friday 8:30 AM‑5 PM.
