Primary Care Nurse Practitioner / Physician Assistant – Part‑Time

Position at Eventus Whole Health delivering integrated primary care to residents of skilled nursing and assisted living facilities.

• Provide primary‑care services in accordance with site‑specific protocols and standard of care for family, geriatric, or internal medicine in long‑term care facilities.
• Review and sign medication orders, DME orders, death certificates, plans of care, PT/OT/hospice orders; address labs within 24 h; manage narcotic prescriptions per policy; complete documentation and submit billing records per published policies.
• Maintain open and positive communication with facility staff and administration; provide verbal/written instruction regarding medications.
• Be knowledgeable of and adhere to Eventus Whole Health standards, policies, legal and regulatory requirements and HIPAA rules.
• Consult the clinical supervisor before refusing a resident, family, or facility request or recommendation regarding resident care.

• Master’s degree in Nursing from an accredited institution and current and valid state Nurse Practitioner license.
• OR
• Graduate from an accredited Physician Assistant program, pass the National Commission on Accreditation of Certified Physician Assistants exam, and hold state Physician Assistant license.
• Specialization in geriatric, family, or psychiatry desired.
