Outpatient Diabetes Educator‑RD (PRN)
We are seeking a registered dietitian to join the multidisciplinary diabetes team at The Christ Hospital Health Network. The role involves designing, coordinating, and implementing comprehensive diabetes care and education in accordance with ADA standards.
Responsibilities- Considering age‑specific physiological, developmental, and cultural/socioeconomic parameters, assess educational and nutritional needs of patients diagnosed with diabetes mellitus.
- Conduct assessments that include medical history, nutrition practices, physical activity, medications, health literacy, readiness to learn, and readiness to change behaviors.
- Assist patients in determining realistic and measurable goals or outcomes consistent with diabetes practice guidelines and appropriate to their state of health, resources, and needs.
- Guide patients in identifying behaviors necessary for goal attainment within a designated time frame.
- Monitor progress toward achievement of predetermined goals.
- Provide Diabetes Self‑Management Training (DSMT) in group and individual settings in accordance with the nine content areas of the American Diabetes Association: diabetes pathophysiology, healthy eating, being active, taking medications, monitoring glucose, prevention/detection/treatment of acute and chronic complications, lifestyle and healthy coping, and diabetes distress and support.
- Provide Medical Nutrition Therapy (MNT) to patients requiring in‑depth guidance and education to achieve nutritional behavioral goals.
- Utilize clinical practice recommendations and standards of care of the American Diabetes Association (A.D.A.) and design the curriculum for DSME.
- Participate in the planning or implementation of educational programs for the community or professional health‑care staff as needed.
- Maintain ongoing communication with other pertinent departments and health‑care providers of the network.
- Communicate clearly, concisely, and accurately while considering others’ needs and beliefs.
- Collaborate with clinical staff to develop plans of care to meet the needs of patients with diabetes.
- Provide up‑to‑date information regarding diabetes and its management to various health‑care groups.
- Document educational sessions and post‑program support plans in the EMR and communicate the diabetes education plan and progress with referring providers.
- Participate as an active member of the multidisciplinary diabetes team.
- Literature favors the registered nurse, registered dietitian, and pharmacist as the key primary instructors for diabetes education and as members of the multidisciplinary team.
- Participate in department meetings and annual advisory meetings.
- Work collaboratively with others to meet customer needs.
- Utilize appropriate journals, books, websites/list servers, and attend seminars, workshops, and conferences; non‑CDEES dietitians must complete 15 hours of continuing education on diabetes topics each year.
- Participate in competency assessment process and self‑evaluation to establish professional goals and development plan emphasizing specialization as appropriate.
- Participate in diabetes‑related professional organizations such as DECA, the professional division of the A.D.A., the American Association of Diabetes Educators (AADE), and ADCES.
- Maintain or work toward certification as a Diabetes Care and Education Specialist.
- Participate in interdepartmental and intradepartmental performance improvement activities related to patient care and service.
- Participate in data collection and development of the core curriculum for ADA recognition.
- Demonstrate competencies addressing the unique characteristics and needs of patients across the life span, including cultural norms, religious preferences, language/communication barriers, and literacy limitations.
- Coordinate scheduling of patients.
- Perform other duties as required.
- Education: Bachelor’s or Master’s degree from an accredited education program of the Academy of Nutrition and Dietetics.
- Experience: General clinical dietetics practice desirable; one year’s experience in practice area preferred.

Skills:
Knowledge of adult learning principles, diabetes mellitus management, and ability to develop and implement effective medical nutrition therapy plans for patients with prediabetes or diabetes; strong verbal and written communication, people, problem‑solving, decision‑making, time‑management, and leadership skills; computer skills in Microsoft Office and EMR. - Licenses &
Certifications:
Active Registration Status with the Commission on Dietetic Registration of the Academy of Nutrition and Dietetics; current license to practice dietetics in the State of Ohio; license to practice dietetics in Kentucky required within six months of hire; certification as a Diabetes Care and Education Specialist (CDCES) preferred; certification required within four years of hire.
