Transition Facilitator (Youth Case Manager)
Greater Cincinnati Behavioral Health Services (GCBHS) invites applications for a Transition Facilitator to provide Community Psychiatric Support Treatment (CPST) and Therapeutic Behavioral Services (TBS) to youths and young adults with severe mental illness in an outpatient facility and community settings.
Base Pay Range$18.11 per hour – $20.63 per hour.

- Provide case management and CPST/TBS services to youths and young adults with severe mental illness.
- Initiate, coordinate, and monitor evaluations needed to identify each client’s needs (living arrangements, financial issues, benefit status, legal status, physical/psychiatric health, activities of daily living, social supports, leisure and recreation).
- Engage clients in identifying and expressing their own needs and re-assess outcomes regularly.
- Maintain knowledge of mental health treatment, basic needs resources, and agency linkages.
- Understand client population characteristics and practice setting; address socioeconomic, cultural, gender, racial, and sexual orientation issues affecting service use; and utilize system resources effectively.
- Master’s degree in mental health, social work, counseling, or related field.
- OR BSW, Bachelor in psychology, criminal justice, or related human services field plus two years of specialized education, training, or experience in one of the specified fields.
- Experience in social services and behavioral health preferred but not required.
- Valid driver’s license, personal vehicle, and insurable driving record required; frequent driving necessary.
