Social Worker II – Tri Health
Part‑Time Day – Good Samaritan Hospital NICU. Provide social support and education to parents of newborns in NICU. Weekend rotation providing social work to maternity patients; may require weekend days at Bethesda North Hospital.
Job OverviewAssess psychosocial needs of patients and families, develop, communicate, and implement comprehensive plans for psychosocial interventions and complex discharges. Serve as resource regarding social systems, institutional needs, behavioral patterns. Collaborate with multidisciplinary team to ensure efficient, appropriate care, promoting smooth transitions and optimizing health outcomes. Provide education and expertise to promote the practice of social work within the hospital, and serve specialist areas requiring an MSW for regulatory or grant funding.
Job Requirements- Master’s Degree in Social Work or Social Science (required).
- Up to 1 year clinical healthcare or community social work experience (required).
- Computer literate.
- Experience in grief and bereavement counseling, alcohol and drug intervention, support for victims of abuse, neglect, and violence.
- Knowledge of payer rules for reimbursement and community resources for underserved populations.
- Licensed Social Worker.
- Apply root cause analysis to readmissions; work with healthcare team, patients, and families to resolve issues.
- Communicate verbally and in writing with multidisciplinary team to advise on plan and patient response.
- Counsel other caregivers on techniques for effective psychosocial intervention and communication.
- Develop and implement plan of care based on assessment, including counseling or crisis intervention; facilitate linkages to community resources and services; revise plan based on patient and family response.
- Promote the profession of social work by providing formal education and teaching rounds; serve as resource and preceptor for new staff.
- Provide on‑call services per departmental policy, including timely response and appropriate follow‑up.
- Respond to referrals and assess psychosocial and complex discharge needs of patients and families using data from medical record, team, family, and other resources.
