Job Overview:
The purpose of this position is to serve as part of the ambulatory treatment team providing direct behavioral health services to patients and families. Duties may include consultation, assessment, individual/family/group counseling/psychotherapy, education, prevention, medical exam room-based intervention, referral and service coordination with other community agencies and programs. Assist with program development and evaluation as directed. Perform other clinical, administrative, educational and/or research duties (commensurate with training and ethical scope of practice) as needed and directed.

- Qualifications:
Master s Degree;
Licensed Independent Social Worker (LISW) OR Licensed Professional Clinical Counselor (LPCC); generalist knowledge of a wide variety of behavioral health conditions and evidence-based treatment modalities to serve diverse groups of patients across the lifespan; able to work in a team-based integrated brief treatment model; up to 1 year experience as Professional Clinical Social Worker or Clinical Consultant.
- Clinical Service: provide behavioral health services including assessment and evidence-based treatment in an ambulatory care setting; conduct clinical interviews, administer and score standardized assessment tools, establish clinical diagnoses, provide psychotherapy/counseling, and maintain supporting documentation. Utilize evidence-based interventions such as motivational interviewing, problem-focused therapy, mindfulness, cognitive behavioral therapy, and acceptance and commitment therapy. Help patients develop skills for disease self-management (improved coping, distress tolerance, stress reduction, relaxation).
- Consultation and
Collaboration:
develop and maintain collaborative relationships with primary care, ambulatory practice team, behavioral health team members, patients and families; provide behavioral health education to staff, patients and families in individual and group settings; work with community mental health providers and support services to deliver a continuum of care. - Case Management and Treatment Coordination: collaborate with the primary care team to provide referrals, case management and follow-up services; engage patients, initiate referrals, address barriers to care and follow-up; use community resources to address psychosocial and behavioral health needs.
- Documentation: provide accurate and timely documentation in Epic, Agility or other authorized EHRs; support program evaluation and fidelity measures as directed.
- Quality Improvement: assist in monitoring the behavioral health program, identify improvement areas; participate in meetings, continuing education, community education, and committees as directed by supervisor.
- Supervisory Support: may provide supervisory guidance to team members, assess supervisee performance, approve intervention and implementation plans; may assume responsibility for supervisee clients’ welfare.
Incumbent must be independently licensed clinical social worker or Licensed Professional Clinical Consultant; experienced in providing consultation, assessment and treatment of behavioral health problems for patients across the lifespan; willing to work in a fast-paced team-based practice.
