Job Responsibilities
- Medication Orders – Evaluate orders based on patient weight, allergy history, and compatibility. Record orders in the patient profile, complete them without error, and prepare for delivery with the next run. Clarify and resolve problem orders, contact physicians or nurses as needed, and pass information to the next shift. Detect all problem medication orders, generate alternatives, and effect changes in patient care.
- Intravenous Preparations – Train pharmacists and technicians in unit dose and IV areas. Prepare all IV preparations (TPN, central lines, antibiotics, chemotherapy, respiratory syringes) according to departmental guidelines, check for errors, and ensure delivery as per policy.
- Chemotherapy Orders – Process chemotherapy orders with dosages and calculations checked by two pharmacists. Use current precautions when preparing chemotherapy (gowns, gloves, chem‑mat), prepare in the appropriate hood, and use calculations indicated on the chemotherapy patient cardex.
- Serve as a Resource – Use up‑to‑date references for drug compatibility, interactions, dosage form selection, and dosing by patient weight/age. Review patient data upon physician request to recommend serum concentration draws. Handle queries professionally, especially over the phone and at the window. Anticipate, analyze, and address conflict situations; take preventative or corrective action. Identify concerns and assist in problem solving. Function in any area of the central pharmacy where an R.P.H. is needed.
Take administrative initiatives when a supervisor is unavailable. Act as an informational resource for the institution.
- Bachelor of Science in Pharmacy or Pharm
D Degree - Pharmacist license in Ohio or within 3 months of hire
- Completion of PGY1 Residency
- Experience in Hem Onc and/or BMT
- Experience with pediatrics
