Tri Health is recruiting for the role of Outpatient Behavioral Health Therapist (LISW, LPCC or Psychologist). This is a full-time position located in Cincinnati, OH.
Job Overview
The purpose of this position is to serve as part of the ambulatory treatment team providing direct behavioral health services to patients and families. Duties may include consultation, assessment, individual/family/group counseling/psychotherapy, education, prevention, medical exam room-based intervention, referral and service coordination with other community agencies and programs. Assist with program development and evaluation as directed. Perform other clinical, administrative, educational and/or research duties (commensurate with training and ethical scope of practice) as needed and directed.

Job Requirements
Master's Degree
Licensed Independent Social Worker (LISW); OR
Licensed Professional Clinical Counselor (LPCC)
Generalist knowledge of a wide variety of behavioral health conditions and evidence-based treatment modalities to serve diverse groups of patients across the lifespan
Must be able to work in a team-based integrated brief treatment model
Up to 1 year experience as a Professional Clinical Social Worker or Clinical Counselor
Job Responsibilities
Clinical Service:
Provide behavioral health services including assessment and evidence-based treatment in an ambulatory care setting. Include clinical interview administration and scoring of standardized assessment tools, clinical diagnosis of behavioral health conditions, psychotherapy/counseling and supporting documentation. Familiar with evidence-based interventions such as motivational interviewing, problem-focused therapy, mindfulness, cognitive behavioral therapy, and acceptance and commitment therapy. Help patients develop skills to facilitate disease self-management (e.g., coping, distress tolerance, stress reduction, relaxation).
Consultation and
Collaboration:
Develop and maintain collaborative relationships with the primary care provider, ambulatory practice team, behavioral health team members, patients and families. Provide behavioral health education to staff, patients and families in individual and group settings. Work with community mental health providers and community support services to develop, provide and support a continuum of care for patients.
Case Management and Treatment Coordination:
Work with the primary care team to provide referral, case management and follow-up services for patients with behavioral health needs. Engage patients, initiate referrals, address barriers to care and follow up. Be familiar with community resources to address psychosocial and behavioral health needs.
Documentation:
Provide accurate and timely documentation in Epic, Agility or other authorized electronic health records to support identification and treatment of patient psychosocial and/or behavioral health needs. Assist in capturing program evaluation and fidelity measures as directed.
Quality Improvement:
Collaborate to monitor the behavioral health program and identify areas for improvement. Participate in meetings, continuing education, community education and committees as directed by supervisor.
Supervision:
May provide supervisory support to other team members (professional guidance, assess supervisee performance, approve interventions and implementation plans) and assume responsibility for the welfare of supervisee clients.
Other Job-Related Information
Independently licensed clinical social worker or Licensed Professional Clinical Counselor with experience in consultation, assessment and treatment of behavioral health problems across the lifespan; willingness to work in a fast-paced team-based practice.

Salary range: $56,000.00–$64,000.00 per year.
