• Work as part of an in‑house therapy team (not outsourced)

• Assist Physical Therapists in implementing individualized treatment plans for residents
• Help patients improve mobility, strength, flexibility, and function
• Encourage and motivate patients during therapy sessions
• Provide education to patients and families on exercises and at‑home care
• Monitor and document patient progress and communicate with the care team
• Contribute to care planning and treatment modifications as needed
• Promote wellness and injury prevention within the resident population
• Ensure all therapy services are delivered in accordance with facility protocols and regulations

• Associate’s Degree from an accredited PTA program
• Current PTA license in the state of Ohio
• Strong communication and interpersonal skills
• Professional, compassionate, and patient‑centered
• Willingness to collaborate with a multidisciplinary team

• Experience in skilled nursing or long‑term care settings
• Knowledge of evidence‑based therapy practices
• Familiarity with electronic documentation system – Net Health
