Behavioral Health Case Manager

Case Manager (Service & Support Administrator), Developmental Disabilities Services (4649-66)

Full-time, Monday – Friday, 9:00 a.m. – 5:00 p.m.

$18.00 per hour – $21.00 per hour

• Provide therapeutic behavioral services to clients by consulting with licensed clinicians to assist with individual needs.
• Develop service plans for individualized support, care coordination, and treatment plan development.
• Offer linkage, interventions, treatment options, restoration of daily life skills, and crisis prevention services.
• Provide services to individual clients or groups of clients.
• Promote a positive and cooperative relationship with all outside contacts.

• Must be at least 18 years of age.
• High school diploma plus ≥3 years of relevant field experience or a bachelor’s or master’s degree in a related field.
• Experience or training with mental or behavioral health conditions.
• Valid driver’s license and reliable transportation.
• Willingness to travel throughout the assigned service area.
