• Associate degree or higher in a related field

• Experience with counseling individuals and/or groups

• Valid Driver's License and auto insurance coverage

• Reliable transportation

• Responsible for planning, implementing, and maintaining a bereavement program that supports the families/caregivers of the deceased for up to one (1) year after the patient's departure.

• Development and implementation of the bereavement plan of care for the bereaved following the hospice patient's death.

• Develop a bereavement plan of care using the bereavement assessment, the risk assessment, input from the IDG, and interaction with the bereaved following the patient's departure.

• Provide bereavement support to hospice personnel coping with work-related grief.

• Provide memorial services, grief support groups, and community education, send mailings, bereavement calls, bereavement visits, and other services deemed necessary.
