Compensation & Benefits
  • Earning potential: $115,000–$200,000 salary based on experience + easily achieved monthly bonuses

Key Responsibilities
  • Deliver person‑centered care using evidence‑based practices
  • Collaborate with interdisciplinary facility teams
  • Conduct assessments, develop treatment plans, and manage follow‑up care
  • Provide ongoing follow‑up and chronic care management in skilled nursing facilities (SNFs)
  • Educate patients, families, and staff
  • Complete timely documentation within 24 hours
  • Ensure compliance with facility, state and federal regulations/protocols
Qualifications
  • Active license as a Nurse Practitioner (NP)
  • Minimum 2 years of post‑certification experience
  • Experience in SNFs, hospitalist roles, emergency departments, or ICU preferred
  • Proficient with EHRs and clinical technology
  • Strong communicator with a collaborative mindset
