We are seeking nurses (RN or LPN) who are skilled clinicians, detail‑oriented, and compassionate. Your role will involve addressing patient needs, responding to emergencies, creating personalized care plans, and effectively communicating patient conditions.

• Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans
• Administer medications and monitor patients for side effects or reactions
• Document any changes in patient conditions and report on changes
• Maintain accurate records of patient care and discuss observations with the supervisor

• Currently licensed Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Ohio
• Previous hospice experience is preferred but not mandatory
• Commitment to excellence in clinical care and customer service
• Excellent communication and interpersonal skills
• Valid driver’s license
• First aid/CPR certification required
