Job DescriptionThe Telephonic Triage RN provides nursing care services to assigned practices and specialties. They complete a biophysosocial telephonic assessment using approved clinical protocols to triage patient needs and provide the best course of action or patient disposition. As a patient/caregiver advocate, the RN acts as a communication link among the patient, caregiver, physician, pharmacy, and hospital using approved system platforms. The RN has strong working knowledge of emergent, urgent, and non-urgent patient needs, utilizes best practices in documentation, and strives to provide first‑call resolution.

- Promotes a positive work environment by collaborating with team members, recognizing colleagues, and participating in peer‑to‑peer coaching.
- Manages in‑basket messages in a timely manner, responds to patient concerns, and achieves first‑call resolution when in scope.
- Ensures all messages sent to providers contain complete and accurate information; implements nursing practice changes resulting from quality activities.
- Opens shifts in a “Ready” mode, minimizing “AUX/Not Ready” time to less than 20% of total time; completes patient hand‑offs to healthcare team using SBAR.
- Utilizes AIDET+P, HEARD Model, scripting, and workflows to create a standard patient experience; proactively handles complaints and escalates grievances.
- Assesses and triages patient status via phone and EHR; utilizes clinical protocols for disposition or advice; reviews documentation and provides education materials.
- Performs activities that maintain specialty/department models, systems, policies, protocols, and cultural norms; supports collaborative relationships among the healthcare team; endorses diversity and cultural competency.
- Associate's Degree in Nursing (Required)
- 2–3 years Clinical Nursing (Required)
- Experience in Acute Care (Required)
- Strong Computer Skills; ability to type and talk simultaneously (Required)
- Knowledge of biological, physical, behavioral, social, and nursing sciences (Required)
- RN – State Licensure and/or Compact State Licensure (Required)
- Basic Life Support (BLS) certification (Required)
