- Take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track.
- Troubleshoot and make sound decisions related to dialysis treatment; partner with SNF staff to coordinate care when needed.
- Perform Direct Patient Care and supervise Dialysis Technicians who assist with care delivery to meet program goals.
- Provide patient education to patients and families in the SNF setting.
- Demonstrate autonomy and critical thinking; apply core nursing skills to assess, diagnose, and implement care plans while monitoring the whole patient.
- Primary work schedule: 3-4 days a week with some travel to other SNF clinics within your region (depending on business and patient needs).
- Alternating Saturdays may be required.
- Current Registered Nurse (RN) license in the state of practice; minimum of 12 months RN experience.
- Associates Degree in Nursing (ADN) from an accredited school of nursing required;
Bachelor of Science in Nursing (BSN) or three-year diploma may be substituted for the nursing degree. - Basic computer skills and proficiency in MS Word and Outlook.
- Previous chronic hemodialysis or acute dialysis experience is preferred but not required.
- PAID HANDS-ON TRAINING PROGRAM; available training through DaVita’s programs.
- Training/Teaching/Healthcare Education background.
- Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN).
