• Possess, as a minimum, an accredited Nursing Degree or a graduate of an approved RN training program and possess a current, unencumbered, active license to practice as an LPN in the State of Ohio.
• Prefer experience in a hospital, acute skilled nursing care facility, or other related medical facility.
• Be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long‑term care.
• Be willing to work harmoniously with other personnel and families and communicate effectively with residents at all behavior or acuity levels.

• Promote residents’ well‑being and satisfaction by supporting resident‑centered care, developing and interpreting standards of care, and guiding the interdisciplinary team as it establishes and implements residents’ plans of care.
• Model and ensure that others provide medical treatment in a manner that honors resident preferences and individuality.
• Administer medications and perform nursing procedures according to established policies and nursing standards of care, ensuring all orders and changes are followed.
• Administer professional services such as catheterization, intravenous therapy, tube feedings, suction, dressing changes, etc.
• Monitor activities of daily living, assess mental, physical, emotional and spiritual well‑being, and encourage resident participation in activities.
• Document assessments and progress notes in the EMR accurately and timely, perform other administrative duties such as completing medical forms, evaluations, studies, and reports.
• Help plan, develop, implement, and evaluate activity/recreational programs for residents.
• Uphold the philosophy of Person‑Centered Care and provide purpose, choice and freedom to residents.
