Job Description & How to Apply Below

Help build specialty endocrine care from the ground up in the Cincinnati area. A growing geriatric endocrinology practice is launching in 12 new nursing home and skilled nursing buildings in Cincinnati and is hiring one to two full-time Nurse Practitioners to open them. You'd bring hands-on diabetes and thyroid care to long-term care residents, serve as the primary clinical decision maker for a portfolio of five to six facilities, and start in brand-new buildings, with no one else's patient panel to inherit.

You'll work Monday through Friday with no weekends or after-hours coverage, and a collaborating physician is always one call, text, or Face Time away.

What Makes This Role Different

A true ground-floor opportunity: you start in brand-new buildings and build your patient panel from scratch. A focused specialty: your visits center on diabetes and thyroid care, not the five or six issues packed into a typical primary care visit. Real autonomy with a safety net: you make the clinical decisions, and a collaborating physician is available by phone, text, Face Time, or email whenever you want a second opinion.

A gradual ramp: you start with one to two facilities and grow to five or six as you settle in. In a new building, initial visits run about 7-10 patients a day until every resident has been seen, then you move into a steady rhythm of about 30 patients a day across your facilities. Follow-up visits average about 15 minutes, with up to 30 minutes for more complex patients.

Facilities are clustered, so you aren't driving more than 30-40 minutes to a building, and a second building on the same day is either nearby or on your way home. A true Monday-Friday schedule with no weekends and no after-hours coverage, plus a flexible start time. Start early and finish early if you like.

Endocrinology experience is a plus, not a requirement: the practice layers its endocrine training on top of a baseline understanding of diabetes and insulin management.

Your salary stays the same when you move from 1099 to W-2 after your first 90 days.

Key Responsibilities .
  • Provide on-site diabetes and thyroid care to residents in nursing homes, skilled nursing facilities, and long-term care centers, including new patient visits and follow-ups.
  • Manage insulin and other diabetes and thyroid medications, and adjust treatment plans based on each resident's progress.
  • Act as the primary clinical decision maker for your assigned facilities, consulting the collaborating physician when you need support.
  • Educate patients and, when patients can't make decisions on their own, their families on the importance of proper diabetes care.
  • Build working relationships with facility leadership, directors of nursing, and nursing staff, and adjust your communication style to what works best at each building.
  • Follow the practice's standardized processes while adapting to how each facility operates.
  • Represent the practice as an ambassador at every facility you serve.
  • Maintain accurate, timely clinical documentation.
Qualifications.

Required.
  • Active Ohio nurse practitioner license.

  • A self-starter who works well independently, with strong critical thinking and problem-solving skills.
  • At least 1 year of experience as an NP in long-term care, skilled nursing, or post-acute care; 2 years is ideal.
