Hybrid role: facility-based therapy and local home care services. Facility services paid hourly; home care services paid per visit.
Monday – Friday, Full Time.
General SummaryProvide physical therapy services to patients with physical dysfunction, including assessment, treatment planning, and implementation. Participate in patient care conferences, program development, screenings, supervision of PTAs and aides, and documentation. Adhere to policies, procedures, and clinical practice standards.
Your Responsibilities- Conduct physical therapy screenings and evaluations to determine patient status.
- Provide treatment consistent with APTA standards.
- Develop and implement treatment plans using strengthening, modalities, and retraining to maximize patient independence.
- Deliver direct patient care in line with productivity standards, using graded activities.
- Check, instruct, assign, and review work of PTAs and aides.
- Maintain accurate documentation for quality, continuity, and reimbursement.
- Participate in patient care conferences as requested.
- Engage in CQI activities to ensure continuous quality of care.
- Promote current standards through continuing education and in‑service programs.
- Coordinate and/or lead in‑service programs for internal or external groups.
- Participate in new program development and company meetings.
- Successful completion of an accredited Physical Therapy program.
- Licensure in Ohio.
- Valid driver’s license.
- Professional judgment and initiative in problem analysis and solution recommendation.
- Initiative and flexibility in the work environment.
- Effective response to patient and family needs.
- Physical ability to bend, position, transfer, and lift patients weighing 80 to over 300 pounds using recommended lifting devices.
