Part‑time Registered Nurse Case Manager

Anticipated Territory: Placerville to Cameron Park

Schedule: Every Saturday and Sunday full days; might consider Saturday/Sunday plus 3 weekdays.

What you’ll do:

• Conduct start‑of‑care (SOC) admissions – OASIS
• Perform follow‑ups and evaluation visits as needed in designated territory
• Make home visits to patients in designated territory
• Reconcile medications, and provide wound care, treatments, and procedures
• Monitor clients’ conditions and report changes to the clinical team
• Collaborate with physicians and agency staff to deliver optimal care
• Accurately document observations, interventions, and evaluations for patient care management
• Initiate and administer IV medication infusions (antibiotics and diuretics) safely and effectively per protocols

To be a success:

• Current and unrestricted CA RN license
• Current BLS/CPR certification
• Valid California driver’s license
• Experience with assessments and SOC – OASIS
• High level of clinical knowledge in home health care
• Experience with high‑acuity patients and interdisciplinary teamwork
• Fundamental case‑management skills
• Experience with IV medication infusions
• Self‑directed, empathetic, critical thinker, strong interpersonal and leadership skills, organized
