All opportunities require regular home‑visit fieldwork.
What you’ll do:- Conduct Start of Care (SOC) admissions – OASIS.
- Perform follow‑up and evaluation visits as needed in the designated territory.
- Make home visits to patients in the territory.
- Reconcile medication, provide wound care, treatments, and procedures.
- Monitor clients’ conditions and report changes to the clinical team.
- Collaborate with physicians and agency staff to deliver optimal care.
- Accurately document observations, interventions, and evaluations.
- Initiate and administer IV medication (antibiotics and diuretics) infusions safely and effectively in accordance with protocols.
Must be available every Saturday and Sunday full days. Preference for additional 3 weekdays.

- Current BLS/CPR certification.
- Valid California driver’s license.
- Experience conducting assessments and SOC – OASIS.
- High level of clinical knowledge in home health care.
- Experience with high-acuity patients.
- Experience working with an interdisciplinary team.
- Case management fundamentals.
- Previous experience with IV medication infusions.
- Proficiency initiating intravenous (IV) therapy.
- Self-directed.
