Full Time Registered Nurse Case Manager, Level 2

What you’ll do as a Registered Nurse (RN), Level 2 Home Health Case Manager:
- Develop, implement, and monitor comprehensive care plans to promote wellness and prevent readmission.
- Lead a team of 2 nurses, providing oversight, mentorship, and training on wound care, blood draws, tube feedings, tracheostomy care, Foley catheters, CHF management, and medication education.
- Conduct thorough patient assessments (Start of Care, Evaluations, Resumption of Care, discharge planning).
- Collaborate with interdisciplinary teams, physicians, and agency staff to coordinate optimal patient outcomes.
- Document clinical notes, orders, and charting in a timely, compliant manner.

To be a success as a Registered Nurse (RN) Home Health Case Manager, Level 2:
- 1–2 years of nursing experience with at least 1 year in home health case management preferred.
- Experience supervising and mentoring nurses highly valued.
- Current, unrestricted California RN license.
- Current BLS/CPR certification.
- Valid California driver’s license.
- Strong clinical knowledge of home health care and high-acuity patients.
- Experience working with interdisciplinary teams and understanding case management fundamentals.
- Self‑directed, empathetic, critical thinker with outstanding interpersonal and organizational skills.
